Trulieve’s third-quarter 2026 results represent its first full quarter as an NYSE-listed company and its first reporting period under a medical-only structure after the June 2026 deconsolidation of Harvest’s mixed operations. These changes complicate direct year-over-year comparisons, yet they provide a clearer view of the core U.S. medical cannabis business. With federal rescheduling of state-licensed medical marijuana to Schedule III and final DEA registrations in place, the quarter highlights how Trulieve’s margins and cash generation are progressing amid supportive regulatory developments.
For the quarter ended September 30, 2026, Trulieve posted revenue of $230.2 million. The headline number declined from $288.2 million a year earlier, but that comparison is skewed by the Harvest deconsolidation. On a like-for-like medical-only basis, revenue rose 4% and topped management’s guidance. I also checked this using Tickeron’s AI Screener to see how the stock compares to others in the industry.
Gross profit reached $148 million, for a 64% gross margin, up from 59% in the prior-year period. Net income attributable to common shareholders came in at approximately $1 million, versus a $27 million loss a year earlier. Adjusted net income was $18 million, or $0.09 per diluted share, broadly in line with expectations.
Adjusted EBITDA totaled $89 million, a 39% margin compared with 36% a year ago. Operating cash flow was $74 million, free cash flow was $45 million, and cash on the balance sheet stood at $327 million at quarter-end.
Investor focus has centered more on earnings quality than the top-line figure. Gross-margin expansion, the shift to positive net income, and strong free cash flow point to greater efficiency in the medical-only business post-restructuring. The results cleared a modest analyst bar, especially on margins and cash generation, underscoring the advantages of Trulieve’s scale in Florida and early traction in Georgia and Pennsylvania. The early repayment of a $65 million mortgage also signaled balance-sheet discipline. Still, the smaller consolidated revenue base and dependence on state-level regulatory moves leave some investors cautious on the pace of future growth.
Looking ahead, the key question is whether Trulieve can maintain its margin improvements while funding growth initiatives. Georgia remains a focal point, with patient counts now above 60,000 and plans underway for a tenth dispensary license plus wholesale distribution to independent pharmacies.
Texas offers longer-term potential, where the company has started building vertical operations and holds royalty-free licenses for Alien Labs and Connected products. Capital spending for these expansions may modestly affect near-term free cash flow, though the solid cash position provides room to maneuver.
On the regulatory side, relief from the 280E tax burden and completed DEA registrations should continue to support profitability. Investors will want to track gross-margin sustainability, promotional spending in Florida, and whether adjusted EBITDA margins remain in the high-30% range as new markets scale.
