Pfizer continues working through a well-documented shift away from pandemic-era products. With Comirnaty and Paxlovid sales normalizing lower, attention centers on whether newer and acquired medicines can sustain growth. The third-quarter update offered a useful checkpoint on commercial execution and the ongoing cost-realignment effort. Momentum in areas such as Eliquis, Padcev, the Vyndaqel family, Nurtec, and Lorbrena now sits at the heart of the investment thesis, while the obesity and oncology pipeline offers longer-term potential. The decision to raise guidance reflected management’s confidence even as loss-of-exclusivity pressures remain.
For the third quarter of 2026, PFE reported revenue of $16.58 billion, essentially flat on both a reported and operational basis versus $16.65 billion a year earlier. The figure exceeded consensus estimates of roughly $16.1 billion. Reported diluted EPS was $0.56, down from $0.62 a year ago, while adjusted diluted EPS held at $0.87, above the analyst consensus range of about $0.72 to $0.73.
Growth came from the non-COVID portfolio, which rose 7% operationally, and from launched and acquired products, which advanced 32% operationally. Eliquis increased 25%, Padcev rose 55%, the Vyndaqel family climbed 12%, Nurtec ODT/Vydura advanced 33%, and Lorbrena gained 43% operationally. These gains were offset by Paxlovid, down 63%, Comirnaty, down 29%, and the Prevnar family, down 10%. Adjusted R&D spending rose 18% operationally as the company invested in oncology and obesity programs. I also checked this using Tickeron’s AI Screener to see how the stock compares to others in the industry.
Pfizer raised full-year 2026 revenue guidance by $1.5 billion at the midpoint to $62.0–$64.0 billion and narrowed adjusted diluted EPS guidance to $2.95–$3.05, up from $2.80–$3.00 previously.
The response to the report was measured despite the headline beats. PFE shares moved only modestly around the release, with some sessions showing a slight decline of roughly 1% even as pre-market indications initially ticked higher. The restrained reaction reflects investor emphasis on the still-flat overall revenue picture and the ongoing drag from COVID-19 products and the Prevnar franchise, rather than the adjusted EPS outperformance. The raised guidance and 32% growth in launched and acquired products were viewed positively, yet markets continue to weigh near-term LOE headwinds and the pace of the portfolio transition.
Looking ahead, several factors will shape PFE’s path. The commercialization of the obesity portfolio stands out as a key variable. Management plans to advance roughly 10 active and planned Phase 3 trials in 2026, including work on the investigational monthly GLP-1 peptide berobenatide, with potential first approvals targeted for 2028.
Oncology execution will also matter. The legacy Seagen portfolio grew 39% year over year in the U.S., and further uptake of Padcev, along with late-stage readouts such as Elrexfio in multiple myeloma and the next-generation CDK4 inhibitor atirmociclib, could strengthen the growth story.
Cost discipline remains on track to deliver nearly all of the targeted $7.2 billion in net savings by the end of 2026, with an additional $2.5 billion expected from 2027 through 2029. Investors will also monitor COVID-19 product guidance of approximately $4 billion for the full year and how the newly appointed CFO approaches capital allocation, dividends, and potential share repurchases.
